Qualifications

• Experience in home-based support and personal care.
• Ability to work autonomously in community settings.

Responsibilities

• Provide personal care and practical support (showering, dressing, meal prep).
• Assist with household tasks and transport to appointments.
• Provide social support and companionship in consumers' homes and communities.
• Follow individual care plans and report changes in health/status.
• Maintain confidentiality and build respectful relationships with diverse clients.
