Job summary

ATSICHS Brisbane is seeking a Community Support Team Member to deliver person-centred support to Aboriginal and Torres Strait Islander people with disabilities, Elders, and community members. The role covers domestic assistance, social support, and community engagement under programs including NDIS, CHSP, QCSS, and Disability Supports.

The position requires experience in disability support or related fields, with a strong focus on culturally safe practices and individual choice.

Qualifications

• Professional or Personal experience in disability support, aged care, or community services.
• Understanding of person-centred and culturally safe practices.
• Current First Aid and CPR certification.
• Knowledge of NDIS and aged care sector regulations.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.

Responsibilities

• Provide tailored support to Aboriginal and Torres Strait Islander people with disabilities to enhance their independence and quality of life.
• Implement individual support plans and report on participant progress.
• Provide in-home support, including light domestic tasks, meal preparation and medication reminders.
• Facilitate social inclusion by supporting clients in attending community groups and activities.
• Conduct scheduled wellbeing checks for Elders, ensuring their needs are being met.

Education

About the Role

The Community Support Team Member is responsible for delivering person-centred support to Aboriginal and Torres Strait Islander people with disabilities, Elders and other community members requiring assistance. This role encompasses a variety of responsibilities, including working alongside our clients to deliver domestic assistance, social support, and community engagement, while promoting individual choice, dignity, and cultural safety. The role works across several programs, including Disability Supports, NDIS, Community Groups, Queensland Community Support Scheme (QCSS), and the Commonwealth Home Support Program (CHSP).

Duties

• Provide tailored support to Aboriginal and Torres Strait Islander people with disabilities to enhance their independence and quality of life.
• Implement individual support plans and report on participant progress.
• Provide in-home support, includinglight domestic tasks,meal preparation and medication reminders.
• Facilitate social inclusion by supporting clients in attending communitygroupsand activities.
• Conduct scheduled wellbeing checks for Elders, ensuring their needs are being met.
• Adhere to workplace health and safety standards, including infection control procedures.
• Participate in team meetings, training, and continuous professional development

• Starting base rate from $36.46to $39.61perhour+ casual loading.

COVID-19 Requirement

• Immunisation history showing you have received two doses of a Covid-19 vaccine is encouraged.

Required

Professional or Personalexperience in disability support, aged care, or community services.

Understanding of person-centred and culturally safe practices.

• Valid driver's licence and access to a reliable vehicle.

Experience working with Aboriginal and Torres Strait Islanderpeople.

Strong interpersonal and communication skills.

• Current First Aid and CPR certification.
• Qualification in Community Services, Disability, Aged Care, or a related field.
• Ability to work independently and as part of a team.
• Knowledge of NDIS and aged care sector regulations.

• Current Working with Children Card (Blue Card) or be eligible to apply.
• NDIS Worker Screening (Yellow Card) or eligibility to apply.
• Driver's Licence.

• Travel between work sites on a regular basis.
