Hiring4Health is seeking a Registered Nurse in New South Wales to deliver home-based community nursing and provide clinical leadership for NDIS and aged care clients in our region. This part-time role covers fifteen hours per week with responsibilities spanning wound care, medication administration, and ongoing client assessment.

You will work closely with the Aged Care and NDIS teams, maintain accurate records, support staff training, and ensure compliance with AHPRA standards in a community

Qualifications

• Current AHPRA registration is required.
• Experience in community nursing is essential.
• Experience with older people or individuals with disabilities is preferred.

Responsabilités

• Provide professional nursing care at clients\' homes, including wound care, injections, catheter management, and clinical assessments.
• Collaborate with Aged Care and NDIS Team Leads to meet care goals per support plans.
• Notify Branch Manager if additional nursing care is needed beyond the support plan.
• Respond promptly to health changes with assessments, reviews or referrals in consultation with the client and family.
• Report changes in client condition or home environment to Aged Care/NDIS Leads and Branch Manager.
• Encourage client independence and self-sufficiency.
• Maintain accurate and comprehensive documentation for each client visit.
• Coordinate with allied health providers to support clients\' health and well-being.
• Provide clinical leadership and staff education on epilepsy, diabetes, mealtime management, medication administration, and manual handling.
• Adhere to Nursing and Midwifery Board of Australia\'s standards and AHPRA\'s ethics and responsibilities.
• Engage in ongoing professional development through training opportunities.
• Participate in continuous improvement activities and planning days.
