Qualifications

• Current AHPRA registration with no conditions.
• National Police Clearance.
• Current NDIS Worker Screening Check.
• First Aid Certificate / CPR.
• Ability to work efficiently under pressure.

Responsibilities

• Deliver high-quality, person-centred nursing care in home or community settings.
• Conduct holistic assessments and develop care plans to promote independence.
• Coordinate and evaluate care plans to achieve optimal client health outcomes.
• Work with multidisciplinary teams and external providers.
• Monitor client health status and escalate concerns as needed.
• Maintain compliant clinical documentation and reporting.
