Job summary

CatholicCare Wollongong is seeking a Registered Nurse to deliver nursing care and case management in clients' homes across Illawarra, with occasional travel to Shoalhaven.

You will join a small, collaborative clinical team, coordinate services across health providers, and support care workers; requires AHPRA registration, experience with complex clients, NSW Driver’s Licence and First Aid.

Qualifications

• Registered Nurse with current AHPRA registration.
• Experience working with complex clients in aged care, disability, or community settings.
• Demonstrated clinical leadership and mentoring capability.
• Current NSW Driver’s Licence.
• Current First Aid Certificate.

Responsibilities

• Deliver direct nursing care and individual case management to aged care and NDIS participants in homes and the community.
• Conduct nursing assessments and develop/implement/an review care plans in collaboration with clients and families.
• Support direct care workers through education and guidance to maintain best practice clinical standards.
• Coordinate services across health providers and community stakeholders to meet changing needs.
