Job summary

The Kent Community Health NHS Foundation Trust is seeking a Community Learning Disability Nurse to join the Learning Disability Team in the West Kent service. The post holder will provide specialist nursing interventions to adults with a learning disability and manage a caseload under clinical supervision, while supervising junior staff and students.

Based in the community, the role works closely with multi-disciplinary teams and families to deliver holistic care.

Qualifications

• Registration with the Nursing and Midwifery Council is required.
• Level 6 degree or equivalent, or experience plus a nursing diploma.

Responsibilities

• Assess, plan, implement and evaluate holistic care linked to care packages.
• Develop integrated care programmes with the client.
• Supervise and develop junior staff and students.
• Work with a multi-professional team to deliver high quality care.
• Provide a range of clinical interventions to support therapy outcomes.
• Manage a daily caseload of adults with learning disability, including complex needs.
• Provide specialist nursing interventions to adults with learning disability.
• Help clients access generic services with clinical support.
