Due to growth, we’re recruiting a Co-Care Manager to lead and support our Perth-based Co-Care Partner team.

The Role

As a Co-Care Manager, you’ll play a key leadership role within PlanCare. You’ll be responsible for the day-to-day coordination, support, and performance of a team of Co-Care Partners, each managing their own client caseload.

This is a hands‑on role that combines people leadership, client oversight, and operational governance. You’ll work closely with your team to ensure clients receive high-quality, compliant, and person-centred support, while also contributing to continuous improvement across the business.

You’ll also maintain your own client interactions as required, particularly during onboarding, complex matters, and escalation scenarios.

This is a full‑time role, Monday to Friday, 9:00am to 5:00pm, based in our Perth office.

What you’ll be doing:
Team leadership and support
  • Lead, coach, and support a team of Co-Care Partners
  • Oversee team workloads, caseload distribution, and performance
  • Provide guidance on complex client, provider, or funding matters
  • Ensure consistent service delivery, governance, and documentation standards across the team
Client oversight and governance
  • Oversee client onboarding, monthly check‑ins, and ongoing support activities
  • Support clients and Co-Care Partners with budgeting and support plan management
  • Review and oversee invoice processing to ensure alignment with client support plans and government guidelines
  • Manage escalations and complex client or provider enquiries
Operations and collaboration
  • Resolve client and provider enquiries via phone and email
  • Build strong relationships with clients, providers, and internal stakeholders
  • Provide feedback and insights to support the continuous improvement of PlanCare systems, processes, and client materials
  • Assist in shaping the direction of the business as we grow and scale
We’re looking for someone who:
  • Has experience leading or supervising a team in aged care, community care, or a similar environment
  • Has strong knowledge of My Aged Care and Home Care Packages, particularly self‑managed HCPs
  • Is confident in supporting clients with questions about their funding, support plans, and services
  • Brings excellent written and verbal communication skills
  • Has a strong person‑centred approach and a genuine commitment to supporting older Australians
  • Has experience in case management and client governance
  • Is organised, detail‑oriented, and comfortable managing multiple priorities
  • Is proactive, collaborative, and not afraid to share ideas or feedback
  • Enjoys working in a team environment and having a laugh along the way
You’ll need to have:
  • Proven experience managing or supervising a team
  • Strong administrative and computer skills with excellent attention to detail
  • Well‑developed problem‑solving skills
  • An NDIS Worker Screening Check or recent Police Check, or willingness to obtain one
  • The right to live and work in Australia
It would be great if you also had:
  • A current and valid nursing qualification
  • Experience in My Aged Care or Community Care
  • A qualification in Aged Care or Community Services
  • Lived or learned experience of the aged care sector
  • Experience using PRODA, the My Aged Care portal, or Services Australia systems
