A supportive healthcare provider in Western Australia is seeking a Co-Care Manager to lead a team of Co-Care Partners. The role involves overseeing client support, team performance, and operational governance. Ideal candidates will have experience in aged care, excellent communication skills, and a commitment to supporting older Australians. This full-time position offers a vibrant working environment with career progression opportunities.
Howdy, we’re PlanCare, a fast-growing, innovative healthcare platform on a mission to transform the Support at Home sector through smart, people-centred technology.
The Future of Aging recently recognised us as the Technology – Business Award Winner 2025, and by the Australian Financial Review as one of Australia’s Top 10 Most Innovative Health Companies, and we’re only just getting started.
At PlanCare, we help older Australians take control of their Support at Home services so they can live independently, confidently, and on their own terms. As we continue to scale nationally, we’re strengthening our leadership and support teams, and that’s where you come in.
Due to growth, we’re recruiting a Co-Care Manager to lead and support our Perth-based Co-Care Partner team.
As a Co-Care Manager, you’ll play a key leadership role within PlanCare. You’ll be responsible for the day-to-day coordination, support, and performance of a team of Co-Care Partners, each managing their own client caseload.
This is a hands‑on role that combines people leadership, client oversight, and operational governance. You’ll work closely with your team to ensure clients receive high-quality, compliant, and person-centred support, while also contributing to continuous improvement across the business.
You’ll also maintain your own client interactions as required, particularly during onboarding, complex matters, and escalation scenarios.
This is a full‑time role, Monday to Friday, 9:00am to 5:00pm, based in our Perth office.
