Job summary

Metro North Hospital & Health Service seeks a Clinical Pharmacist (including Virtual Ward) to provide a broad range of clinical pharmacy services across the Community and Oral Health (COH) directorate on a rotational basis. The role focuses on transitions of care, Hospital in the Home, and telehealth services, working within a multidisciplinary team to ensure patient-centred care and medication safety.

• Experience delivering clinical pharmacy services within hospital settings.
• Ability to provide medication information and education to patients and carers.

Responsibilities

• Be part of the multidisciplinary team delivering pharmacy services aligned with COH model of care.
• Support transitions of care including inpatient rehab, Hospital in the Home, and community-based services.
• Provide medications information and education to patients and carers.
• Deliver clinical education and supervise students with support from senior staff.
