Job summary

Nepean Blue Mountains Local Health District (NBMLHD) seeks a Clinical Nurse Specialist to lead continuing care coordination at Hawkesbury District Hospital. You will coordinate discharge planning, support patient flow and engage multidisciplinary teams to ensure seamless care transitions.

The role requires current RN/Midwife registration (AHPRA) and post-registration experience in clinical areas, with strong knowledge of aged care and primary health care systems.

Qualifications

• Current registration as a Registered Nurse/Midwife Division 1 with the Australian Health Practitioner Regulation Agency (AHPRA).
• Post-registration experience in clinical care, discharge planning and coordinating patient journeys.
• Knowledge of aged care systems and primary health care coordination.

Responsibilities

Hawkesbury District Health Service is a teaching hospital in Windsor NSW, providing acute, community and allied health services for the local community and surrounding districts. The service provides 24-hour emergency, medical, surgical, diagnostic, maternity, neonatal, paediatric, palliative, intensive and coronary care services.

Make a meaningful impact on patient outcomes as a Continuing Care Coordinator, providing expert care coordination and guiding patients through every stage of their healthcare journey. In this role, you will work collaboratively with patients, families, multidisciplinary teams and external providers to coordinate discharge planning, transfer of care and referrals to a range of health, rehabilitation, transitional and community services.

Using your advanced clinical skills, you will undertake comprehensive assessments, support patient flow, maintain high-quality clinical documentation and contribute to quality improvement initiatives that enhance service delivery and patient experience.

What you will bring to the role

• Current registration as a Registered Nurse/Midwife Division 1 with the Australian Health Practitioners Regulation Authority AHPRA.
• Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate.
• Demonstrated working knowledge of services both within the inpatient and community health setting including My Aged Care, ACAT, GPs, Community Health and chronic disease management.
• Demonstrated knowledge and understanding of Patient Flow, Discharge Planning, Aged Care Assessment Team and Primary Health Care.
• Demonstrated ability to utilise problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care.
• Demonstrated advanced and effective communication skills evidenced through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
• Knowledge and understanding of person-centred care, evidenced based practice and quality improvement Demonstrated understanding of and ability to practice in accordance with legislation, policies and within the scope of practice of a Registered Nurse
• Current NSW Driver Licence
