The Queensland Government's Mackay Base Hospital is seeking a Clinical Nurse Consultant (Connected Community Pathways) to join the Connected and Integrated Care Unit on a permanent basis. You will lead a multidisciplinary team delivering culturally safe care for Aboriginal and Torres Strait Islander communities and others at risk of Type 2 Diabetes Mellitus.

Qualifications

• The Clinical Nurse Consultant will lead a multidisciplinary team for Aboriginal and Torres Strait Islander and vulnerable populations with or at risk of Type 2 Diabetes Mellitus in persons aged 15 years and older.
• Role aligns with health equity strategies and the organisation's plan to improve Aboriginal and Torres Strait Islander health.
• Position is in the Mackay Hospital and Health Service District within primary health care and prevention settings.

Responsibilities

• Lead a multidisciplinary team delivering culturally safe care for Aboriginal and Torres Strait Islander communities and others at risk of Type 2 Diabetes Mellitus.
• Work within primary health care and prevention settings to reduce ED presentations and potentially preventable hospitalisations.
