Job description

What you'll be doing

The CNC Grade 2 Aged Care / Dementia is an expert nursing practitioner supporting the education, professional development and leadership of community nursing and allied staff ensuring provision of effective, safe, and appropriate evidence based aged and dementia care, enhancing, and extending staff knowledge and skill base through the provision of expert assessment, consultancy, care planning and education. The role will also liaise, coordinate, and plan care with other health professionals and relevant community services, ensuring appropriate support for patients referred to NSHNS with complex aged care / dementia issues.

Candidates will need to meet the following criteria :

  • Registered with Authority to Practice with Australian Health Practitioner Regulation Agency. Minimum five years full time post graduate experience with 3 years full time equivalent in the specialty field. In addition the applicant must have an approved post graduate qualification (minimum Grad Diploma / Certificate level) or evidence of working towards Masters level qualification.
  • Demonstrated clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
  • Demonstrated ability to undertake quality improvement and research activities that influence and enhance clinical practice and implement change.
  • Current NSW Driver License.
