About the Role:

We are looking for a confident and adaptable clinical leader who thrives in a collaborative environment and is passionate about supporting older people to achieve their rehabilitation goals.

The Transitional Therapy and Care Program (TTCP) provides up to 12 weeks of goal-focused therapy and care services for older people within their own home or at the TTCP residential unit. The program is designed to maximise recovery and functional independence following hospitalisation, helping older people regain confidence and avoid premature admission to residential aged care.

Under limited direction of the TTCP Manager, the Clinical Lead provides high-level clinical leadership, expertise and coordination across aged care and rehabilitation services within an interdisciplinary model. The role is responsible for leading efficient referral and admission processes, coordinating high-quality and safe service delivery for consumers accessing TTCP, and supporting staff in the management of complex clients. Working collaboratively with internal and external stakeholders, the Clinical Lead drives service access, patient flow and discharge planning, while fostering strong professional relationships and promoting evidence-based practice to achieve optimal consumer outcomes.

The successful candidate will have a flexible and adaptable approach to work, well-developed critical thinking skills, and the ability to make sound, evidence-based decisions with a solutions-focused mindset. They will possess excellent interpersonal and communication skills, enabling them to build effective relationships and confidently engage with a broad range of stakeholders across the health and aged care sectors.

A merit pool may be established from this recruitment process and will remain valid for 12 months. This pool may be used to fill similar roles/ future vacant positions at the same level across CHS.

Please note prior to commencement successful candidates will be required to:

  • Comply with Canberra Health Services Occupational Assessment, Screening and Vaccination policy.
  • Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.

Note: This is a part-time (29.4 hours per week) temporary position available for 12 months with the possibility of extension and/or permanency. The full-time salary noted above will be paid pro rata for part-time hours.

Note: Under the ACTPS employment framework you may be placed in another role where required for the efficient administration of the service.
