About the role

Juniper is currently seeking passionate and experienced Registered Nurses to join our team as Clinical Care Partner to cover SOR suburbs from Bentley to Mandurah. This permanent full-time role offers the opportunity to lead clinical care delivery in our community-based aged care programs, Support at Home, TCP, and NATSIFACP.

Key Responsibilities
  • Lead the clinical components of Juniper’s community-based aged care services (Support at Home, TCP, and NATSIFACP), with a strong focus on customer-centred and integrated care.
  • Oversee the development and delivery of individualised services based on each customer’s assessed clinical needs.
  • Collaborate with customers, families / guardians, and health professionals to ensure care plans are comprehensive, safe, and aligned with individual goals and preferences.
  • Provide clinical leadership and oversight across multiple programs, supporting and guiding non-clinical staff.
  • Provide clinical guidance and support for escalation and risk management.
  • Completing clinical assessments.
  • Monitor and respond to emerging clinical risks, initiate reassessments, and ensure timely escalation and follow-through.
  • Ensure compliance with Aged Care Quality Standards, relevant legislation, Juniper and NDIS Codes of Conduct, and internal policies and procedures.
  • Lead clinical assessments and support compliance with Strengthened Aged Care Quality Standards.
  • Advocate for inclusion, cultural safety, healing-informed and trauma-aware practices—particularly in rural, remote, and Aboriginal and Torres Strait Islander communities.
  • Work closely with care partners, allied health professionals, administrative teams, and families to deliver integrated and effective care plans.
  • Support governance, training, and workforce capability-building across Juniper’s community programs.

What You’ll Bring
  • Current AHPRA registration as a Registered Nurse
  • Minimum 5 years’ clinical experience in aged care, community care, or transitional care
  • Strong knowledge of SAH, TCP, NATSIFACP, and Strengthened Aged Care Quality Standards
  • Experience mentoring multidisciplinary teams and supporting non-clinical staff
  • Proven ability to manage clinical governance, audits, and continuous improvement initiatives
  • Commitment to culturally safe, trauma-aware, and healing-informed care
  • Advanced clinical assessment and care planning
  • Excellent communication, coaching, and mentoring abilities
  • Strong documentation and digital literacy (Alaya Care experience desirable)
  • Collaborative, proactive, and customer-focused approach
  • Ability to navigate complex care environments and drive quality outcomes
