Bupa Aged Care in Melbourne is seeking a Care Manager to lead and oversee the delivery of person-centred care for residents, supported by a capable clinical team and strong governance. You will partner with the General Manager to ensure high standards of assessment, care planning and evaluation, while driving continuous improvement.

The role demands nursing registration, leadership capability and a passion for compassionate, quality aged care.

Qualifications

• Bachelor of Nursing with APHRA registration.
• Post-grad qualification in aged care is highly regarded.
• Strong leadership, communication and interpersonal skills.
• Person-centred care mindset.
• Passion for nurturing a strong team environment.
• Proven track record delivering quality clinical care in aged care or acute settings.
• Excellent clinical assessment and triaging skills.
• Experience fostering a reporting culture balanced with investigative skills.
• Understanding of Resident Rights and Aged Care Standards and Outcomes.

Responsibilities

• Oversee the daily clinical operations of the care home to ensure residents receive the best possible care.
• Lead the care home clinical team including coaching, mentoring, staffing allocations and performance management.
• Review clinical incidents and complaints, providing solutions to reduce risks.
• Lead compliance with AN-ACC assessments.
• Manage person-centred care planning and evaluation for residents, including family conferences.
• Foster a strong, collaborative team environment.
• Prepare and conduct clinical governance audits.
• Provide insights and innovative solutions for continuous clinical improvement.
• Oversee medication management in line with resident needs and regulations.
• Liaise with allied health professionals to optimise resident care.
