Eastern Health is seeking an experienced Registered Nurse for Chronic Rehabilitation in Melbourne's eastern suburbs. The role involves coordinating care for clients across hospital and community settings within a multidisciplinary team, delivering comprehensive assessments, care planning and discharge planning.

Ideal candidates will have a Bachelor of Nursing, NMBA registration via AHPRA, at least five years of clinical experience, and a strong focus on patient-centred care and chronic disease

Formación

• Bachelor of Nursing leading to registered status in Australia.
• Registration with NMBA via AHPRA.
• Minimum five years of relevant clinical experience.
• Current Victorian Driver's Licence.
• Strong written and verbal communication with consumers, families and health professionals.
• Proficient computer literacy and health informatics experience.
• Able to problem-solve in complex situations.
• Post-graduate in a relevant specialty.
• Experience with chronic disease management and care coordination.
• Exposure to KPI monitoring and reporting.

Responsabilidades

• Coordinate and manage clients in the Chronic Rehabilitation program within the multidisciplinary team.
• Provide specialised nursing services across hospital and community settings.
• Conduct comprehensive clinical assessments, develop and review care plans.
• Support safe transitions and timely discharges according to patient needs.
• Deliver education and support to patients, families and staff on care coordination and chronic disease management.
