Job summary

The Mala’la Health Service seeks a Chronic Disease ITC Program Care Coordinator to lead care planning for clients with complex chronic conditions in Maningrida and surrounding homelands. The role is Darwin-based with FIFO: 2 weeks in Maningrida and 2 weeks in Darwin, with accommodation and flights subsidised.

Applicants must hold current AHPRA RN registration, BLS, Medicines Management, and have at least 5 years nursing experience, plus experience in remote Aboriginal communities.

Qualifications

• Current AHPRA Registration and Practicing Certificate as a Registered Nurse.
• Current Basic Life Support Certificate.
• Medicines Management module within the last 12 months.
• Vaccination training or equivalent knowledge.
• Minimum 5 years of broad nursing experience post-registration.
• Experience in remote or culturally diverse settings is preferred.

• Develop high-quality care plans in partnership with the multi-disciplinary team.
• Provide culturally safe, competent and comprehensive care.
• Develop and maintain strong referral pathways.
