Qualifications

• Experience in service delivery to clients in the community.
• Desirable nursing or allied health experience.
• Evidence of current Police Check.
• NDIS Screening where applicable.

• Assess, refer and support re-enablement for clients.
• Collaborate with health and community teams to deliver care.
