UnitingCare Children, Young People and Families (BlueCare) is seeking a Care Partner to coordinate person-centred care for clients using aged care funding. You will assess needs, develop individualised care plans and work with families and multidisciplinary teams to ensure best outcomes.

You will travel in the Gympie region to support clients in their homes and communities, manage care budgets and funding, and help clients navigate the care system with a compassionate approach.

Formación

• Certificate IV in a Health or Community Service discipline, essential.
• Current Australian Driver’s Licence, essential.
• Vehicle for work that is roadworthy and insured, essential.
• Knowledge of client-centred community care funding streams.
• Ability to collaborate with clinical and care teams.
• Aged care legislation and quality requirements knowledge.
• Strong communication, interpersonal and problem-solving skills.
• Ability to satisfy probity checks.

Responsabilidades

About the role:

As a Care Partner, you’ll coordinate person-centred care and services to support clients to achieve the best possible outcomes from their Aged Care Service funding.

• Assessing client needs and developing and reviewing individualised care plans
• Coordinating services and care transitions with clients, families and multidisciplinary teams
• Managing client care budgets and available funding
• Monitoring changing needs and risks, escalating to clinical teams where required
• Reviewing care plans, services and outcomes
• Supporting clients to navigate the care system and access appropriate services

Role requirements:

• Certificate IV in a Health or Community Service discipline - Essential
• Current Australian Driver’s Licence - Essential
• Roadworthy and insured (Third Party Property minimum) vehicle for work - Essential
• Experience and knowledge of client-centred community care and Community Funding streams
• Ability to collaborate with clinical and care teams to coordinate care and exercise professional judgement
• Knowledge of relevant aged care legislative, regulatory and quality requirements
• Strong communication, interpersonal and problem‑solving skills
• Ability to satisfy the relevant probity checks required by legislation or policies – Essential
