A leading home care provider seeks an experienced Care Coordinator in Far North Queensland to enhance community support services. This position emphasizes hands-on client care, case management, and team collaboration, requiring strong qualifications in aged care and effective communication skills.

Role Overview
Our Far North Queensland office continues to grow and we are looking for another experienced and dedicated Care Coordinator to support our growth. The successful candidate will preferably have a strong background in Community Aged/Disability Care. The role itself is responsible for service development, coordination and delivery, including ongoing client assessments and the development of care plans that deliver flexible and personalised in-home support. The successful applicant will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care.
This role does require some flexibility as some weekend supports are likely to be required, with this time to be offset as reasonable through the week.
This key role will focus on case management, customer service, personal care work where required and general administration.
