Role Overview

Our Far North Queensland office continues to grow and we are looking for another experienced and dedicated Care Coordinator to support our growth. The successful candidate will preferably have a strong background in Community Aged/Disability Care. The role itself is responsible for service development, coordination and delivery, including ongoing client assessments and the development of care plans that deliver flexible and personalised in-home support. The successful applicant will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care.

This role does require some flexibility as some weekend supports are likely to be required, with this time to be offset as reasonable through the week.

This key role will focus on case management, customer service, personal care work where required and general administration.

Qualifications
  • Minimum of Certificate IV Aged Care;
  • Diploma in Nursing with a current AHPRA registration highly desired;
  • Current First Aid & CPR; and
  • Driver's License.
Preferred Skills
  • Good understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary);
  • Good understanding of the hospital discharge environment;

  • Proven leadership abilities, professionalism, a good reputation & respect for our staff, clients and their families;
  • It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills; and
