Job summary

ATSICHS Brisbane is seeking a dedicated Care Coordinator Nurse to support patients with chronic illness. You will provide care coordination and work closely with clients to improve health outcomes in Aboriginal and Torres Strait Islander communities.

The role requires a Bachelor in Nursing and full unconditional AHPRA registration, with strong communication and collaboration skills. This is a permanent full-time position in Brisbane, offering a starting salary around AUD 78,447 plus 12% super.

Qualifications

• Minimum Bachelor in Nursing.
• Full un-restricted Registered Nurse AHPRA registration.
• Previous demonstrable experience in relevant nursing field.
• Demonstrated ability to meet targets and performance outcomes.
• Ability to support a continuous improvement model for achieving outcomes.

Responsibilities

• Provide care coordination to assist in the provision of chronic disease management.
• Establish and maintain relationships with key external stakeholders conducive to holistic health and social support needs.
• Foster a culture of continuous improvement and innovation within the Primary Health Support Services, integrating culturally safe practices.
• Ensure effective implementation and maintenance of ATSICHS policy and procedures in line with AGPAL Accreditation Standards.
