Join a supportive MDT, conduct comprehensive assessments, support discharge planning, and contribute to equipment provision and patient independence.

• Conduct comprehensive assessments on the Medicine Escalation Ward.
• Evaluate and provide necessary equipment to support patient independence and rehabilitation.
• Develop and implement discharge planning to ensure safe and timely transitions.
• Participate in board rounds to coordinate care with multidisciplinary teams.
• Ensure high standards of occupational therapy practice in line with regulations.
