Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) Case Manager for Detroit-area field-based care. You will deliver comprehensive in-home assessments, coordinate care plans, and lead a team of LPNs and HHAs to support patients at home.

Flexible part-time schedule with productivity targets and mileage reimbursement. The role emphasizes high-quality documentation, OASIS accuracy, and building strong relationships with discharge planners and physicians to drive outcomes.

Qualifications

• Active Michigan RN license.
• Reliable transportation and valid driver's license.
• Home health/OASIS experience preferred.

Responsibilities

• Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC).
• Provide high-acuity patient care, including wound care.
• Develop and manage the plan of care with physicians; supervise LPNs and HHAs on your cases.
• Represent Adaptive to patients, families, and the referral sources.
