BRAMS is seeking Elder Care Connectors (Aboriginal Identified) to join the team in Broome, WA. Reporting to the Community Support Services Manager, this role supports Aboriginal Elders and their families to understand, access, and navigate aged care services they are entitled to.

This is a relationship‑focused role that involves walking alongside Elders, supporting them through My Aged Care processes, helping them prepare for assessments, coordinating services, and advocating for culturally safe and appropriate aged care supports.

You will work closely with Elders, families, assessors, service providers, and internal BRAMS teams to support continuity of care and positive ageing outcomes.

Key responsibilities include :
  • Engaging with Elders and families to raise awareness of aged care services and entitlements
  • Supporting Elders with My Aged Care registration, assessments, reviews, and service navigation
  • Assisting with goal setting, care coordination, and follow‑up
  • Supporting Elders to access and attend appointments and services
  • Making internal and external referrals to ensure appropriate supports are in place
  • Advocating for culturally safe and responsive aged care services
  • Documenting activities and case notes accurately in line with BRAMS requirements
  • Working collaboratively with the Elder Care team and broader BRAMS services

Is This You?

This is an Aboriginal Identified role, open to applicants who identify as Aboriginal and / or Torres Strait Islander.

You’ll be someone who is approachable, community‑focused, and confident building relationships with Elders. You do not need formal qualifications – BRAMS welcomes applicants with lived experience, community knowledge, and a willingness to learn on the job.

You’ll demonstrate :
  • Experience engaging with Aboriginal Elders, families, or community members
  • Understanding of Aboriginal culture, values, and community dynamics
  • Strong communication and relationship‑building skills
  • Confidence working independently while contributing to a team
  • Basic computer skills and ability to complete documentation
  • Organisational skills and the ability to manage priorities

Experience in aged care, community services, disability support, My Aged Care navigation, or working within an Aboriginal Community Controlled Health Organisation will be highly regarded.

You will also require :
  • A current Australian driver’s licence
  • A current or willingness to obtain :
  • Working With Children Check (OCHRE Card)
  • NDIS Worker Screening Check
  • National Police Clearance
