Job summary

APM Communities is seeking a Clinical Aged Care Assessor in Australia. The role involves conducting comprehensive assessments of older adults to identify needs and connect them with services that support independent living.

You will travel daily within your service area, collaborating with families and carers, and ensuring compliance with Department of Health contracts. A supportive, multi-disciplinary team and ongoing training are offered.

Qualifications

• 1 year+ experience with older Australians and client assessments.
• Tertiary health-related qualification with professional registration (e.g. AHPRA).
• Proficient in Word, Excel, Outlook and client management systems.

Job Description

As a Clinical Aged Care Assessor, you'll play a crucial role in identifying the needs of older adults and connecting them with services that help them live independently with the highest quality of life.

Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it’s needed most

Your Role:

• High-Quality Assessments: Conduct thorough Aged Care Assessments to identify the needs of older adults.
• Service Referrals: Facilitate referrals to essential services that support independent living.
• Promote Functionality: Actively promote the retention and maximization of function and capability in daily living.
• Engage Stakeholders: Work closely with families and carers, recognizing them as key stakeholders.
• Compliance: Ensure all assessments are conducted in line with Department of Health contractual requirements.

What You Bring:

• At least 1 year working with older Australians, including client assessments and/or service delivery.
• Tertiary education in a health-related discipline together with professional registration with relevant association(e.g. AHPRA)
• Proficiency in Word, Excel, Outlook, and client management systems.
• Current Australian driver’s licence, comprehensively insured vehicle, and the ability to travel within your service area.
