Job summary

BaptistCare NSW/ACT - Aged Care in Forster, NSW is seeking a part-time Care Assessment Coordinator to lead AN-ACC processes, pre-admission assessments and funding outcomes within the Residential team at Kularoo Centre.

You will work with the Residential Manager and Funding Consultants to support residents’ care journeys while upholding high standards of aged care and service quality. This role offers flexible arrangements and a values-driven workplace.

Qualifications

• Registered Nurse qualification and AN-ACC knowledge desirable.
• Strong interpersonal and communication skills.
• Proficiency in MS Office and clinical documentation systems.
• Ability to lead, train, and support staff.
• Excellent time management and problem-solving skills.

Responsibilities

• Coordinate the AN-ACC process and pre-admission assessments.
