Job summary

The Department of Health Tasmania in Burnie, Tasmania, is seeking a Clinical Nurse Specialist (CNS) for the Rapid Access Service within the Older Persons Mental Health Service. The CNS will provide advanced nursing assessment, short-term intervention, and care coordination to support older Tasmanians to remain safely in their usual place of residence and reduce avoidable hospital presentations.

Qualifications

• Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.
• Current Driver's Licence.

Responsibilities

• Provide advanced specialist nursing assessment, intervention, and consultation for older people with complex mental health concerns, BPSD, and acute changes in clinical presentation.
• Collaborate with a multidisciplinary team to ensure timely assessment, triage, short-term intervention and care coordination.
• Work with residential aged care homes, general practitioners, acute hospitals, aged care providers, families and community services to deliver integrated, person-centred care.
