Job summary

NSW Justice Health & Forensic Mental Health Network is seeking an Aboriginal Health Worker/Practitioner for Primary Care in Bathurst on a temporary full-time basis (maternity leave cover). The role focuses on delivering culturally safe health care to Aboriginal and Torres Strait Islander people within a multidisciplinary team at Bathurst Correctional Centre.

Qualified Aboriginal Health Practitioner or Health Worker with relevant certificates and AHPRA registration is required.

Qualifications

• Aboriginal Health Practitioner: Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and is registered with AHPRA.
• Aboriginal Health Worker: Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care; transition to Practitioner possible after registration.

Responsibilities

• Provide flexible, holistic, and culturally sensitive health care services to Aboriginal and/or Torres Strait Islander people.
• Collaborate with nursing staff and health teams to deliver timely health care.
• Focus on prevention and management of chronic disease for Aboriginal clients, including those transitioning from custody back into the community.

Your Qualifications

We are looking for a qualified Aboriginal and Torres Strait Islander Health Professional that:

• Aboriginal Health Practitioner: Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and is registered with the Australian Health Practitioner Regulation Agency (AHPRA)
• Aboriginal Health Worker: Holds or aspires to hold a minimum Certificate III qualification in Aboriginal Primary Health Care. Aboriginal Health Workers willing to obtain Cert IV in Aboriginal Primary Health Care (Practice) will be supported to transition to an Aboriginal Health Practitioner position upon completion and registration with AHPRA.
• Eligible to drive in NSW and the ability to travel as required.

The Role

As an Aboriginal Health Practitioner, Primary Care, you will work as a key member of our multidisciplinary health centre team to provide flexible, holistic, and culturally sensitive health care services to Aboriginal and/or Torres Strait Islander people and apply key strategies for face-to-face patient care.

There is a focus on prevention and management of chronic disease for Aboriginal clients and supporting them on their journey throughout custody back into community, particularly those with chronic or complex health conditions.

You will work collaboratively with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations and other relevant health and community groups to ensure the delivery of seamless, timely and effective health care to Aboriginal clients.

The Location

Bathurst Correctional Centre is a mixed-security facility for male offenders located in the centre of Bathurst, 1 hour from Orange and the Blue Mountains, and 210 kilometres west of Sydney. The complex serves as a reception prison for western NSW.

On-site you’ll find a close-knit clinical team led by a passionate & enthusiastic Nurse Unit Manager that encourages the development of staff. Working within the Primary care team, you’ll enjoy fantastic training & education opportunities all the while working alongside Mental Health, Drug & Alcohol, Population Health, and Aboriginal Chronic Care teams.
