Job summary

Unite Recruitment is assisting a leading Aboriginal Community Controlled Health Organisation to recruit for a Full-Time role based in Toowoomba. The position focuses on person-centred care, supporting clients to access services, engage with treatment, and improve health outcomes in the Darling Downs region.

The role is identified for Aboriginal and/or Torres Strait Islander applicants, with Monday to Friday hours, no weekends, and a competitive salary with salary packaging and strong benefits.

Qualifications

• Minimum Certificate III in Aboriginal and Torres Strait Islander Primary Health Care (or nearing completion).
• Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Health Practitioner) or willingness to work towards.
• Current CPR certification and a valid Open C Class Driver Licence.
• Ability to obtain and maintain a National Police Check and Blue Card.
• Experience in Aboriginal Medical Service or community-controlled health organisation.

Responsibilities

• Assist with health assessments, including 715 Health Checks.
• Support chronic disease management, care planning, and follow-up.
• Assist GPs and Nurses with clinical procedures and treatment room activities.
• Build strong relationships with Aboriginal and Torres Strait Islander clients and communities.
• Promote culturally safe, person-centred care and health promotion initiatives.
• Navigate health services and support clients to attend appointments.

Key Responsibilities

• Assist with health assessments, including 715 Health Checks

• Identify as an Aboriginal and/or Torres Strait Islander person


• Minimum Certificate III in Aboriginal and Torres Strait Islander Primary Health Care (or nearing completion)


• Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Health Practitioner), or willingness to work towards


• Demonstrated experience delivering primary health care services, or a strong desire to build a career in the field


• Strong understanding of health issues impacting Aboriginal and Torres Strait Islander communities


• Demonstrated ability to work effectively with Aboriginal and Torres Strait Islander people and communities


• Strong communication and interpersonal skills


• Ability to work effectively within a multidisciplinary team


• Ability to manage workload and prioritise competing demands


• Current Open C Class Driver Licence


• Current CPR certification


• Ability to obtain and maintain a National Police Check and Blue Card


• Experience in an Aboriginal Medical Service or community‑controlled health organisation


• Experience supporting chronic disease management and health assessments


• Experience using Best Practice or similar clinical software
