About you:

In your new role you will:

  • Empowering consumers to make informed health care decisions through consultation, education and information sharing.
  • Act as a cultural liaison to advocate, liaise, network and refer to appropriate Non-Government Organisations, Aboriginal and Torres Strait Islander Liaison Officers and the Multidisciplinary Team in collaboration with and under direction of Registered Nurse.
  • Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander Rapid Response Falls Service clients by conducting timely home visits, case management/conferencing, follow-up and referrals to relevant health services/programs.
