Job summary

Cape York Partnership invites applications for an Aboriginal and Torres Strait Islander Health Practitioner to join Ngak Min Health’s multidisciplinary team in Gordonvale. You will deliver culturally safe primary health care, health checks, and client education within your scope of practice.

Qualifications

• Qualification as an Aboriginal and Torres Strait Islander Health Practitioner and AHPRA registration.
• Understanding of culturally safe and responsive health care for Aboriginal and Torres Strait Islander people.
• Knowledge of primary health care, preventative health, chronic disease management and health promotion.
• Strong communication and interpersonal skills.
• Ability to work effectively within a multidisciplinary health care team.
• Current First Aid and CPR certification, or ability to obtain these before commencement.
• Current Queensland driver's licence, or the ability to meet travel requirements.
• Ability to meet applicable pre-employment screening and Vaccine Preventable Disease requirements.

Responsibilities

• Supporting the prevention, early identification and management of acute and chronic health conditions.

• Undertaking screening, immunisation and preventative health activities where appropriately qualified and authorised.
• Supporting referrals, appointments, investigations and follow-up care.
• Working collaboratively with GPs, nurses, allied health professionals and other members of the health team.
• Maintaining accurate, timely and confidential clinical records.
• Advocating for culturally safe and appropriate care and helping clients overcome barriers to accessing health services.

Ngak Min Health is seeking an Aboriginal and Torres Strait Islander Health Practitioner to join our multidisciplinary health team in Gordonvale.

This is an opportunity to contribute to culturally safe, person-centred primary health care while supporting Aboriginal and Torres Strait Islander clients, families and communities to achieve positive health and wellbeing outcomes.

Ngak Min Health delivers culturally responsive primary health care services, particularly to students and families of Djarragun College and Cape York Girl Academy, Cape York Partnership staff and communities throughout the Cairns southern corridor.

The Role

As an Aboriginal and Torres Strait Islander Health Practitioner, you will work closely with GPs, nurses, allied health professionals and other members of the Ngak Min Health team to provide coordinated and culturally appropriate care.

You will support the delivery of primary health care, chronic disease management, preventative health, health promotion and client education, while helping clients understand and navigate health services and participate in decisions about their own health and wellbeing.

About You

We are looking for someone who is passionate about Aboriginal and Torres Strait Islander health and can build respectful and professional relationships with clients, families and colleagues.

To be considered for this position, you will have:

• A qualification as an Aboriginal and Torres Strait Islander Health Practitioner, including Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice, or another qualification recognised for registration.
• Current AHPRA registration with the Aboriginal and Torres Strait Islander Health Practice Board of Australia.
• An understanding of culturally safe and responsive health care for Aboriginal and Torres Strait Islander people.
