We are currently looking for a Care Coordinator to join our Aged Care Team reporting to our Clinical Governance Manager.
Your key responsibilities will include:
Champion person-centred care by working alongside older people to understand their goals, strengths, preferences, and cultural needs, ensuring all supports reflect what matters most to them.
Coordinate holistic, high-quality services that promote independence, wellbeing, and dignity—partnering with clients, their families, and service providers to ensure seamless, respectful, and timely support.
Support individuals to manage their aged care package funding by providing clear, empowering guidance that helps them make informed choices, optimise their budget, and maximise the support available to them.
Regularly review, assess, and monitor services to ensure they remain aligned with the person’s needs, aspirations, and changing circumstances—advocating for adjustments when required.
Build strong, trusting relationships with clients, carers, and community partners, fostering open communication and collaborative decision-making.
Maintain accurate, strengths-based documentationthat reflects the person’s journey, progress, and evolving support needs.
Respond proactively to changes or emerging needs, ensuring supports remain flexible, safe, and responsive.
Travel within the region as required to provide face-to-face support, home visits, and community engagement in a timely, compassionate manner.
Participate in a rotating on-call roster,offering reassuring, solution-focused guidance when urgent support or coordination is required.
Promote wellbeing, empowerment, and inclusion by connecting people with local opportunities, community groups, cultural supports, and services that enrich their quality of life.
What you Will need to Bring
A genuine passion for supporting older people to live with dignity, independence, and connection, guided by a deep respect for individual choice and cultural identity.
Strong person-centred practice skills, with the ability to listen deeply, build trust, and empower people to take the lead in their own care planning.
Experience in coordinating aged care, disability, health, or community services,with the confidence to navigate service systems and advocate for the best outcomes.
Sound understanding of aged care funding models (such as Home Care Packages or similar) and the ability to guide people in maximising their supports and managing budgets effectively.
Excellent communication and interpersonal skills, enabling you to work collaboratively with clients, families, carers, and multidisciplinary partners.
Well-developed organisational and time-management abilities, with the capacity to juggle multiple priorities while maintaining warmth, clarity, and attention to detail.
Ability to conduct assessments and service reviews that are strengths-based, holistic, and responsive to changing needs.
Confidence to work both independently and as part of a supportive team, contributing to a positive, solution-focused workplace culture.
Comfort with travelling across the region, engaging with people in their homes and communities.
Willingness to participate in a rotating on-call roster, responding calmly and professionally to urgent or after-hours needs.
Commitment to continuous learning, cultural humility, and reflective practice to ensure the highest quality of support.
Proficiency with digital tools and record-keeping systems, ensuring accurate, timely, and person-centred documentation.
Qualifications and Requirements
Certificate III or IV in Aged Care, Individual Support, Community Services, or a related field (or willingness to work towards one).
Qualifications in Case Management, Social Work, Nursing, Allied Health, or Community Services are highly regarded.
Current First Aid and CPR certification (or willingness to obtain).
