We are seeking a Registered Nurse for the Post Acute Rehabilitation unit to support patients recovering from illness, injury, or surgery within a multidisciplinary team. The role requires a Bachelor’s degree in Nursing, DOH RN licensure or eligibility, and 2–3 years of post-acute rehab experience.

Qualifications

  • Bachelor’s degree in Nursing or equivalent.
  • Valid DOH license as Registered Nurse or eligibility to obtain one.
  • 2–3 years experience in Post-Acute Rehabilitation Unit.
  • BLS and ACLS certifications required; PALS is an advantage.

Responsibilities

  • Adheres to hospital policies, regulatory standards, and maintains confidentiality.
  • Delivers comprehensive nursing assessments and individualized care plans for post-acute rehabilitation patients.
  • Provides advanced nursing care for medically complex patients, including ventilated or severely disabled patients.
  • Administers medications and assists with complex medical procedures per guidelines.
  • Promotes a safe, culturally sensitive environment and supports family involvement in care.
  • Documents care accurately and communicates with the multidisciplinary team for continuity of care.
  • Participates in quality improvement, audits, and staff development activities.
  • Supports scheduling and ongoing learning within the team.

The Registered Nurse – Post Acute Rehabilitation is responsible in providing specialized nursing care to patients recovering from illness, injury, surgery, or neurological conditions, supporting rehabilitation, functional independence, and safe discharge through collaboration with the multidisciplinary team.

Responsibilities
  • Adheres to M42’s policies, procedures, and regulatory standards, maintaining professionalism and confidentiality at all times.
  • Delivers comprehensive nursing assessments and individualized care plans for patients, with a specialty focus on long-term care to transitional care settings.
  • Provides advanced nursing care, close monitoring, and support for medically complex, ventilated, or severely disabled patients, ensuring best practices and patient safety.
  • Administers medications and assists with complex medical procedures in accordance with clinical guidelines and unit protocols.
  • Promotes a safe, culturally sensitive environment that maximizes patient quality of life and supports family involvement in care.
  • Documents care accurately and communicates effectively with the multidisciplinary team to ensure continuity and escalation of care.
  • Participates in quality improvement, audits, and educational initiatives to enhance patient outcomes and service delivery.
  • Supports staff development through supervision, feedback, and engagement in performance evaluation, scheduling, and continuous learning.
Qualifications
  • Bachelor’s degree in Nursing or equivalent.
  • Valid medical license from Department of Health (DOH) as Registered Nurse or eligibility to get one.
  • 2-3 years experience in Post-Acute Rehabilitation Unit.
  • BLS and ACLS certifications required; PALS is an advantage.
  • Strong critical‑thinking and decision‑making skills under pressure.
