Qualifications

• Required: Bachelor’s in health information management or relevant field or Diploma with 3 years additional experience.
• Desired: Bachelor’s degree in a related field.
• Certification in Medical Transcription is preferred.

Responsibilities

• Accompany physicians during patient visits to document interactions in electronic health records.
• Prepare and assemble medical record documentation and charts for physicians.
• Transcribe patient histories, physicals, procedures, labs, and observations as dictated.
• Ensure accuracy and timeliness of all medical reports and medico-legal documents.
• Clarify unclear information and verify data against lab and radiology results.
• Submit transcribed reports to physicians for review and integration into records.
• Operate transcription equipment and troubleshoot as needed.
• Maintain header/footer correctness and proper spelling, punctuation, and grammar.
