Job summary

Mediclinic Parkview Hospital Dubai in the United Arab Emirates is seeking a Care Counsellor to support high-risk and non-compliant patients with chronic conditions. You will facilitate their onboarding into the Care Gaps Program, promote treatment adherence, and guide them along a patient-centered care journey with compassionate communication.

The role requires a nursing background, a Bachelor’s or Diploma in Nursing, and substantial experience in chronic disease coordination.

Qualifications

• Bachelor's degree or Diploma in Nursing or related clinical field.
• 23 years experience in chronic disease coordination, care navigation, or clinical administration.
• Experience with Electronic Health Records; Trak Care knowledge is a strong plus.
• Fluency in English; Arabic an advantage.
• Experience with coordinating patient onboarding and care journeys.

Responsibilities

• Initiate patient onboarding and guide them through the Care Gaps Program with clinical support.
• Identify, prevent and manage risks to ensure patient safety.
• Foster positive patient experiences and document all education and interactions.
• Record communications per company standards and regulatory requirements.
• Coordinate with nursing leadership for patient care planning.
